Rapid weight loss raises gallstone risk in everyone. GLP-1 inherits some of that risk because of how effectively it causes weight loss.

The mechanism

Rapid weight loss changes bile composition: bile gets more cholesterol-saturated and gallbladder emptying slows. Both encourage stone formation.

How common

Trials suggest gallstone or cholecystitis events occur in 1–2% of GLP-1 patients vs <1% on placebo — meaningful but not overwhelming.

Symptoms to watch for

Pain in upper-right abdomen, especially after fatty meals. Pain radiating to the right shoulder blade. Nausea + jaundice (yellow skin/eyes) = urgent.

How we reduce risk

Maintaining 1.4 g/kg protein intake and including some healthy fats (nuts, ghee, olive oil) keeps the gallbladder contracting normally.
