Documentation
Carry your latest prescription (printed) and a brief letter from your doctor stating diagnosis + medication. Most airports never ask; if they do, you're prepared in 10 seconds.
Carry-on, never checked
Cargo holds can freeze. Your pen rides in the cabin in an insulated pouch with a cold pack. Both TSA and Indian airport security allow medical cold packs.
Time-zone confusion
If your once-weekly day shifts (e.g. travel across 8 time zones), keep it on a fixed Indian-time anchor for the first week, then adjust by ±24 hours one cycle later. Don't double-dose to 'catch up'.
At your destination
Hotel fridges often run warmer than home fridges. If you're staying long, ask for the mini-bar (usually colder) or use a portable insulated travel case with replaceable ice packs.
